• NDIS Access Assessment and Report (if not an existing participant)

• Functional Capacity Assessment (FCA)

• Plan Review Report 

• Assistive Technology Assessment and Report

• Minor Home Modification Assessment and Report

• Complex Home Modification Assessment and Report.
